Print the form, complete all the details and sign the form.

Staple the receipts to the form so each one is visible

and they DO NOT obscure the form and hand them to the treasurer.

Alternately you can complete the form, scan the form and receipts yourself and email the image to the Treasurer.

IMPORTANT:

In both cases, ensure ALL the receipt details are visible and they do not obscure the details on the form.
